On 21st January 2025, SHINE 2Europe hosted the online International Workshop on the Design and Implementation of Inclusive Clinical Studies, bringing together more than 50 active participants from diverse fields. This event explored the critical topic of inclusivity in clinical research, through the lens of four key projects: RadioVal, Liveration, AI4HF (Horizon Europe), and READI (Innovative health Initiative) – all of which place clinical studies at their core.
Designing inclusive clinical studies remains a complex task, often challenged by time, human, and financial limitations. Such constraints risk placing an overwhelming burden on the professionals responsible for developing these studies. However, failing to prioritise inclusivity poses an even greater danger: the marginalisation of underrepresented groups, both in research and in access to optimal medical care.
The workshop gathered a broad audience, including researchers, healthcare professionals, AI and IT developers, ethicists, regulators, policymakers, social scientists, and patients. Together, they examined the main challenges to fostering inclusivity in clinical studies, discussed potential solutions, and identified the relevant stakeholders essential for driving change.
Four high-level speakers presented the featured projects, offering valuable insights into how diversity, equity, and inclusion have been integrated into the design phase and how these principles will continue to shape their implementation.
To encourage deeper reflection, participants were divided into three break-out rooms, each dedicated to discussing specific domains such as ethical, legal, social, technical, and organisational challenges. Within these focused groups, the participants worked collaboratively to prioritise the most pressing issues, identifying actionable solutions and determining the key actors who can support their realisation.
Several significant challenges emerged during the discussions, including:
- Health, digital, and traditional literacy: ensuring that all potential participants can understand and engage with clinical studies.
- Diversity, equity, and inclusion training: providing targeted education for all stakeholders, especially clinical and research teams.
- Flexibility in study protocols: adapting methods to foster broader participation.
- Geographical access: addressing the urban-centric nature of trials that often excludes rural populations.
One particularly urgent challenge highlighted was the limited time and funding allocated for trials and clinical studies — a barrier that significantly affects their successful execution.
Proposed solutions included applying innovative digital tools and routine data collection methods to enhance patient selection processes and promote equity.
